A1: a bit of research ahead of time helps. Finding eating options at hotel and nearby and even calling them to confirm is a good idea.
Also carrying some non perishable acceptable items.

A1. One important thing is to learn how to ask about ingredients in the local language(s)to make sure you are understood! I'm largely vegetarian so I learn how to say "no meat?"

A3. I often think about this. If I were invited for a meal at someone's home, I can imagine circumstances in which I'd eat what I was served before giving offense. Usually I'd just try to eat around my restrictions, eat side dishes, not make a big deal of it.
